&lt;p&gt;At this point and time, for claims that have been submitted and additional medical information is required, the only ways to submit is through fax and mail correct? We fax attachments for patients multiple times, and are told BCBSIL haven&amp;#39;t received them. Some of these claims are months to a year old, only to be told over and over again the fax hasn&amp;#39;t been received.&amp;nbsp;&lt;/p&gt;

&lt;p&gt;How can I have my account actually closed such that I stop getting charged? I have not even lived in Illinois in six months.&lt;/p&gt;
