How do I submit a super bill from my therapist for reimbursement?
Hello, You can submit your bill along with the claim form found here - Medical Claim Form - Domestic (bcbsil.com) - to the address on the form or through the Message Center on your Blue Access for Members account on our website, BCBS Login (bcbsil.com), if this is available for your plan. ~ KW
