How can I get a copy of a claim form from earlier this year?

I need a copy of a claim detail regarding some services by Kabafusion in Jan and Feb of 2023. I have misplaced the copy I received during that time frame. Can I access those online or will that require you to mail me a hard copy?

I have copies of claim details sent to me last week, which show that I don't have any balance to pay to Kabafusion. However, I have received bills from Kabafusion showing that I owe around $1300.00. I don't understand and I want to double check against the claim details from Jan, Feb and Mar.
