My weight and wellness doctor submitted an exception for Saxenda, since my plan does not cover weight management medications, and was denied. What is the best way to submit a plan exception for approval?

Is there a set of data that needs to be provided to possibly get an approval for weight management medication. My BMI is over the base for getting weight management medications and other metrics that would need to be met.
