Hello, You can submit a claim to us for these services. You can find the claim form at www.bcbsil.com/.../medical-claim-il.pdf. Please make sure you attach all documentation to the claim and it can be submitted by mail at the address listed on the claim form, or you can submit it online through Blue Access for Members by sending a secured message and using the attachment icon. You can find the login for Blue Access for Members on the front page of our website at www.bcbsil.com. ~ HC
