Is weight loss medication covered or is there a stipulation that you have had to been in a weight loss program for some length of time? Doctor prescribed Wegovy but was denied by insurance.

I can't find anywhere in the plan where weight loss drugs require you to have been in a weight loss program. According to pharmacy it is a requirement. I took Wegovy prior and it was approved.
