Transition related services

I'm in the process of getting a bilateral mastectomy as part of my FTM transition, but I was told it wouldn't be covered by my plan. All other transition related services have been covered for me in the past, why is it suddenly an issue?

How can I find out about this same thing. I am getting ready to start HRT in April and having to pay out of pocket for this will my insurance cover the top surgery?
