How do I get the past results of Well On Target?

How do I get the past results of Well On Target? They give us a sheet that compares this year's results to the previous, but I would like to see them online. I was told that BCBSIL keeps all the history. I have been doing the annual Well On Target healthy checkup for 3-5 years now.
