Hello, You can submit your claim to the address located on the claim form here - https://www.bcbsil.com/PDF/formfinder/medical-claim-il.pd, you can fax it to (800)-279-7419, or you can send it as an attachment through the Message Center on your Blue Access for Members account on our website, www.bcbsil.com. ~ Kayla
