Expanding Quality Health Care to Underserved Communities in Chicago’s Neighborhoods

Esperanza Health Centers  serves the city of Chicago’s southwest side and offers much needed health services to its residents. Families in Esperanza's service area experience significant economic, educational and health disparities. The area has been designated as a Medically Underserved Area as well as a Health Professional Shortage Area by the Health Resources and Services Administration.

Blue Cross supports Esperanza’s mission to continue offering quality health care to families in southwest Chicago neighborhoods. With our grant, Esperanza Health Centers will launch the Chicago Safety Net Learning Collaborative Sustainability Plan. The purpose of this collaborative is to improve all pieces of their day-to-day work and to address problems preventing the Centers from providing quality health care to low-income and uninsured patients.

Esperanza, which means “hope” in Spanish, wants to give exactly that to its community. Esperanza envisions a healthier community by offering the best medical care to the community residents regardless of immigration status, race, ethnicity, income or if they are uninsured.

All their full-time providers and clinical staff are bilingual and bicultural, so the Centers are able to offer services and programs carefully tailored to the needs and culture of the community it serves.

The services offered at Esperanza Health Centers include the following:

Family Medicine
Primary care is offered to adults. The staff will focus on diagnosing, treating and managing chronic conditions like asthma, diabetes, high cholesterol, high blood pressure and obesity. Esperanza’s adult health providers also have experience in geriatric medicine, pain management and weight loss medicine. Annual exams, immunizations, employment physicals, HIV and sexually transmitted disease (STD) testing, and counseling services are also offered.

Pediatrics
Pediatrics offers immunizations, TB tests, obesity counseling, school physicals and more. After-hours pediatric service is also available, which helps families avoid unnecessary ER visits.

Behavioral Health Consulting
Behavioral Health Consultants offer services to help patients who deal with anxiety, depression or other emotional or medical concerns. The consultant is able to provide immediate consultation and schedule additional consult sessions if needed.

Women’s Health/Obstetrics
Services include annual exams, breast exams, mammograms, Pap smears, prenatal care and HIV and sexually transmitted disease (STD) testing and counseling, as well as free pregnancy testing. The Centers offer an exceptional standard of care for women in the community, including diagnostic and early treatment procedures for cervical cancer.
