Is an annual physical exam and a mammogram covered under this insurance? Or do I have to pay full price for this preventative care?

I just received a bill for the lab work and the mammogram from my last annual physical exam. I was lead to believe that theses things were part of preventative care and would be covered by this insurance. I'm 51 years old and theses are things that I need to stay healthy and aware as I age.
