How can I unenroll myself from my father's BCBSIL Plan?

I have been enrolled under my father's BCBSIL plan, but I have recently moved with my mother  to North Carolina. My parents are divorced. I just turned 18 this year...I need to unenroll myself from BCBSIL as I have been approved for Medicaid here in North Carolina.  I am trying to reach out to my father to ask him to unenroll me from his employer's BCBSIL plan. I was not able to contact him. How can I unenroll myself from my father's BCBSIL Plan? Thanks in advance...
