I have a PPO through my mother's workplace and am trying to get FTM top surgery covered. I have searched online for the specific wording needed in letters from endos therapists etc, and who exactly I need letters from, but the only document I can find with some of that information is from 2014. The 2018 version of the document is missing some of the information in the 2014 document that is more specific about the wording and language needed. Is there somewhere where I can view what I need to get this covered? Thanks in advance!
