Where can I get to, to find this info so I can print it out?
I would like to print out what is covered for seeing a naturopath vs physician or LPN
These are the following questions I was recommended I ask.
Does my plan cover Naturopathic doctors? I believe yes per a phone call a month ago
Do I need a referral to see one? I believe no, per a phone call a month ago
Is there a limit on the number of visits each year?
Is there a maximum dollar limit per family member on coverage annually?
Is there a maximum percentage per family member on coverage annually?
What do I have left for maximum if there is a limit?
Do they cover office visits?
Is there a maximum dollar limit for specialists - massage therapists, physical therapy, acupuncture, or any other specialists?
Does the insurance plan cover lab test and results including allergy tests?
What is my deductible to see a naturopath?
Do I have co payments to see a naturopath? Would this be $40
