Make sure you’re getting the most from your plan by reviewing your coverage each year.

To help you understand any changes to your plan and your benefits, you have access to two documents – Annual Notice of Change and Evidence of Coverage.

What is an Annual Notice of Change?

The Annual Notice of Change or ANOC, is a document that outlines benefit changes that current members will see in the next contract year.

Here’s some samples of information you might find in your ANOC, if they are happening:

  • The health plan that sponsors the plan may change the name of the plan
  • The plan charges for premiums, deductibles, copayments, and/or coinsurance may change, covered prescription drugs, or cost sharing changes*

What is an Evidence of Coverage?

The Evidence of Coverage or EOC, is a document that explains complete coverage for the plan in which a member is enrolled.

The information that you will find in your EOC includes:

  • Important phone numbers and contact information
  • Your medical benefits chart (what is covered and what you pay)
  • What to do if you have a question
  • Definitions of important words

When will I receive these documents?

As a Medicare member you will receive your ANOC via mail by September 30th. Other member materials such as the EOC, pharmacy directory and provider directory can be easily accessed online at: The changes and benefits that are outlined in the material that you receive in September will become effective the first of the new year.
