3-D Mammogram NOT being covered 100% NOW!!!

Why was my DIAGNOSTIC 3-d Mammogram covered 100% 4  years ago but now you are sticking me with an $800.00 "portion you may owe" ?  NOTHING has changed as far as the coding for the mammogram from then until now!  I have a personal and family history of breast issues / cancer so what it your reasoning?

    I also received a bill stating the routine mammogram was not covered and applied to my deductible.  I tried calling the insurance company, but because my company goes through HealthConnect360, they wouldn't talk to me.  The member services I contact through HealthConnect360 do not get back to me.  I thought the ACA law required the insurance companies to pay for these screenings without a deductible or copay!  Why is this not covered?
