Looking for a Health Insurance Form?

Use our Form Finder for Members to locate health insurance forms when you need them. Need to file a claim, order a refill prescription, or make a change to your existing policy? It’s easy to search for and download a form.  

Here are just a few commonly used forms you may be looking for:

  • Claim forms for medical, prescription drug, dental or vision plans.
  • Mail-order forms for home delivery of new and refill prescriptions.
  • HIPAA privacy forms, including the Confidential Communications Request Form, Request Personal Health Information (PHI) Records Form and Standard Authorization Form.
  • Enrollment/Change in Coverage forms.

The type of form you need will depend on your specific plan coverage.

To get started with Form Finder, here are a few helpful tips:

  1. On bcbsil.com, in the top menu go to For Members > Forms and Documents and choose Form Finder.
  2. To narrow your search results, choose a topic from the categories on the left or type keywords in the search box.
  3. With each form listed, use the down arrow next to the form title and a description will appear below.
  4. Click on “Download” to open the form in a new window. You can save the form if needed.

Not finding what you are looking for with the public website Form Finder? Members can also log in to their member account to find forms and a whole lot more. Once logged in, go to Support > Forms and Documents. The forms and documents you can access there will be determined by the health care plan you are enrolled in.
